We are thrilled you are considering Monsignor Slade for your family! Applications are submitted online using the link below. Our admissions staff is here to offer any guidance you may need as you complete the application requirements listed below.
To apply, please submit the following:
- $100 Application Fee (per child; non-refundable)
- Copy of Child's Birth Certificate
- Copy of Child's Baptismal Certificate (if applicable)
- Parish Registration Form (if applicable; for Catholic tuition rate)
- Most Recent Report Cards
- Most Recent Standardized Test Scores (Grades 3 - 8)
- Current IEP and/or 504 Accommodation Plan (if applicable)
ALL application requirements must be submitted before an applicant can be considered for acceptance.
